Notice Type: Combine Synopsis/Solicitation Notice Number: VA263-13-Q-0479 The Department of Veterans Affairs, Network Contracting Office 23, Minneapolis, Minnesota, has a requirement and is seeking quotes for gift (credit) cards to be given to research study participants, NAICS Code: 522210, Federal Supply Classification: R708. Minneapolis VA Medical Center's Research Service is requesting to purchase a quantity of 432 gift cards with a face value of $50 each to be given to research study participants as an incentive for research study participation. This research study is titled "Strengthening VA Implementation of PTSD Treatment through Family Involvement" and has been approved by Health Services Research and Development and by the Institutional Review Board (IBR). Also, requested is to have each gift card customized with the study project logo. The gift cards will have a strict non-reloadable dollar limit of $50 dollars each and cash-outs at ATMs will not be allowed. The gift cards must be accepted by all merchants for purchasing general merchandise, with the exception of cigarette/tobacco and alcohol purchases (if possible). The gift cards will be customized with the study project logo. If customization fees will apply, such fees will be included in the quote response. If initial loading fees will apply to the gift cards, such fees will be included in the quote response. Unused gift cards will be allowed to be returned for a full refund to the VA. If processing/service fees will apply on unused cards, such fees will be included in the quote response. Include a gift card implementation timeframe in the quote response (for example: gift cards will be ready for delivery to VA within five weeks from the receipt of a signed VA purchase order). This notice will be effective Friday, 1/25/2013 and will close Friday, 2/8/2013 at 4:00 pm central time.
