The Naval Special Warfare Group 4 (NSWG-4) requests responses from qualified sources capable of providing a Licensed Practical Nurse (LPN) Health Systems Surveillance Administrator. Performance shall be In Accordance With the below Statement of Work. This requirement is FOB Destination.STATEMENT OF WORK: 1. Perform Medical Reception and Clerical Duties to include: a. Vital Signs, b. Electrocardiogram, c. Forms Preparation, d. Entry and Exit Logs, e. Patient Triage. Perform material control and inventory of the Medical Reception area. Operate, maintain and archive to Naval Standard the Shipboard Automated Medical System (SAMS). Act as a Provider Stand by for Female Examinations. Be Versant with Actively Enforce the Standards Health Insurance Portability and Accountability Act (HIPPA). Perform patient education as required. Correctly safeguard and maintain appropriate levels of security and confidentiality of data and information relating to personnel. Maintain, account, dispense and document over-the-counter medications at Naval Special Warfare Group Four in accordance with BUMED Directives. Conduct formal check-in/out of Naval Personnel within the NSWG-4 Claimancy. Receive, screen, and transfer incoming telephone calls to the Medical Officers and staff. Be familiar with PBX style Business Phone Systems. Track, manage and report attainment on all OPNAV commanders programs. To include but not limited to: HIV, Dental Readiness, and Occupational Health AVIP/SVP. Maintain detailed knowledge of current TRICARE Scheduling standards and requirements, including the DOD Patient Advocacy Programs. Perform administrative support as required by the Medical Officers and Chief Petty Officers to include, but not limited to, the typing of letters, memoranda, monthly reports, data collection and updating of recall bills and physical security logs. Be versant with Navy Correspondence Manual. Train, educate and certify Naval personnel in First Responder and Basic Life Support Skills as detailed by the American Heart Association Curriculum. Act as the custodian of record for archived Medical Records and documents in accordance with DON Records Disposition Manual. Enter provider orders for personnel into CHCS as directed/as required. Be familiar with and adhere to DOD requirements for SIPRNET usage. Act as Departmental Primary Web Master and Maintain currency of information on NSW Medical web pages. Draft and submit for release Naval Radio Messages (RMG). Hours: 07:45 to 15:00. Lunch Break is 30 minutes. Two 10 minute breaks per three hours of work. Be versant and competent in all functions of MS Office. Perform data entry, upkeep and produce periodic reports from NSWIMS/NSWIMPS III Personal Qualifications Program (Medical Module). Perform occupational monitoring and surveillance to standards for shore based facilities detailed in NAVOSH 5100 series publications. Manage and report as directed, periodic status of immunizations for operational forces as detailed in BUMED 6000 series instructions. Operate, track and report NSWG4 Medical Managers as directed. Past Performance information: The quoter shall submit, as part of its quote, information on three previously performed contracts or on-going contracts within the past three years that are similar to the statement of work/item description in the solicitation performed for Federal, State or local Governments, and for commercial firms. Information shall be limited to the name and address of the organization for which the services were performed and the number (phone, fax or Internet) of a contact for each contract listed. The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources including sources outside of the Government. Quoters lacking relevant past performance history will not be evaluated favorably or unfavorably on past performance. However, the quote of a company with no relevant past performance history, while not rated favorably or unfavorably for past performance, may not represent the most advantageous quote to the Government and thus, may be an unsuccessful quote when compared to the responses of other quoters. The quoter should provide the information requested above for past performance evaluation, or affirmatively state that it possesses no relevant directly related or similar past performance. The assessment of the quoter?s past performance will be used as a means of evaluating the relative capability of the responder and other competitors to successfully meet the requirements of the RFQ.
