Community Health Representative (CHR) and Resource and Patient Management System (RPMS) Patient Care Component (PCC) Training

The curriculum shall be evidence-based and culturally appropriate for American Indian and Alaska Native communities. The contractor shall provide Patient Care Component (PCC) training for Community Health Representatives (CHR), that support the Resource and Patient Management System (RPMS) CHR PCC system. Contractor will need to conduct quarterly assessments of which Areas are requesting the training and provide culturally-appropriate technical assistance.
