The purpose of this Amendment No.1, is to extend the Closing Date of Request for Proposal (RFP) No. 522-03-040 for the Procurement of Medical Equipment. Due date is extended to February 13, 2003 at 3:00 pm Honduran time. Following are the respond to questions raised under the RFP No. 522-03-040: QUESTION AND ANSWERS: 1-Is the original response date of January 31st being pushed back? Response: Yes, Due Date has been extended to February 13, 2003 at 3:00 pm. 2- What dimensions and/or type of fiber glass/heavy duty plastic chairs are you seeking? Response: Chairs needed must be very stong. Example: To be able to support 300 pounds withouth breaking. 3- What is the desired thickness of the conductive litter pad? Response: Same response of question No. 2. 4- Since this is a USAID funded project, does the geographical code 000 apply to this RFP? Should the product be manufactured in the US only? Response: Geographic Code is: ?000? USA only. 5-The specification for item # 8 - Balance Beam Pediatric Scale is asking for "Capacity 361 Lb", which is probably a misspelling. Could you, please, clarify what capacity is acceptable? Response: Pediatric Scale: The correct capacity is 36 Lb. instead of 3611lb. 6- Similar products have been specified in the Solicitation IFB 97-050 in 1997. Could you, please, provide us with the model of Soap Dispenser which was purchased at that time? Also, could you, please, indicate the incumbent on the previous procurement? Response: Soap Dispenser specifications are clear. 7- Finally, the solicitation does not specify how many copies of the proposal are required. Could you, please, let us know if one copy and one original will suffice? Response: Three (3) hard copies of your proposal can be acceptable. 8-The RFP is asking for the prices CIP American Embassy in Honduras. Do you intend to award only one contract to one supplier or the items will be split among many suppliers? Response: USAID/Honduras intents to award, multiple awards or only one resulting from this solicitation. 9- Another additional question is about US Flagship. As you probably know, the non-US flagship is less expensive than the US. However, in the solicitation there is no indication whether we should use US or non-US flagship. USAID regulations and FAR are requiring the use of US Flagships when moving the goods out of the country. Will these regulations apply to this RFP? Response: Yes. 10. There is no specification as to the load of the shelves, and whether the shelves may be shipped in bulk or in sets, and whether the assembly will be required in Honduras. Response: We prefer that shelves must be assembled, however you can offer all types of choices. 11- In the RFP's description at www.eps.gov, some dimensions are defined by question marks (ex: "Stainless steel litter, dimensions 76?L X 23?W X 32?H") - does this mean that shelves should be approximately 76"x23"x32"? Response: The shelves requested should be approximately 76"x23"x32". 12- a. Ref. your solicitation above need more specs on the otoscope sets their are a wide varity to choose from. Response: The othoscope specs. are very clear as stated in Lot No. 4 b. Shelving samething, do you need them on casters,or wire shelving? Response: Metal Shelves. We need them without casters c. Resucitators manufacture in mind? Response: None in mind. Please follow specifications as stated in Lot no. 5 13-We have received notice of the combined solicitation but would like to know if we can bid for some items, not all. Response: USAID/Honduras, intends to issue multiple awards (if necessary) or one resulting for this solicitation. Quotations can be made partially (by lots) or complete (all lots quoted), but lots needs to be provided with the exact quantity as requested in the Technical Specifications of the RFP, no less/more. 14- Can we quote part of the RFP or must it be "all" ?? Response: Same as response of question no. 13. 15- Could you please clarify the capacity for item #8? BALANCE BEAM PEDIATRIC SCALE, Heavy duty, lever system. The capacity says 3611b x 1/8 oz Does this mean 11 lbs x 1/8 oz? Or 36 x 1/8 oz? Response: The correct capacity is 36 Lbs. instead of 3611. 16-Can you help me on the shelving? Is it on casters,wired,s/s,or do you have a manufacture on this one? Response: Please refer to response in question no. 12. 17- Do you have the Model # on the Welch Allyn otoscope sets? Welch Allyn has 3.5volt or 2.5volt? Response: Any model of Welch Allyn Otoscope. Welch Allyn has 2.5 volt. 18-Model # of the resucitator? Response: Resucitator?s specifications are clear. 19- Model # soap dispenser? Response: Same as the above. 20-Model # Dri-Clave? Response: Same as the above. 21-How do I locate the Instructions to Offerors-Commercial Items, 52.212-2 mentioned in the RFP? Response: To locate instructions on Commercial Items, please refer to the Federal Acquisition Regulations (FAR) in the following internet website: http://www.acqnet.gov/far/current/html/FARMTOC.html scroll all way down until you find the SubChapter H-Clauses and Forms and click Part 52.2 Text of Provisions and Clauses to make your own search. 22-Can I quote on my letterhead or is a special document required? Response: Yes, your letterhead paper to present your quote can be used. 23-What is your timeframe? when do you intend to award? Response: After receipt of quotations and technical and cost evaluations has been done, we intend to award within 45 days aproximately. 24- Is another color other than light green for the plastic chairs acceptable? Response: Any light colors are prefered. Please offer options. 25- Is the stretcher requirement fixed or hydraulic( move up and down)? No 26- Where is a complete copy of provision 52.212-3, as mentioned above certifications and warranty in the solicitation? Response: Please refer to same reply on question No.4. 28- We have received notice of the combined solicitation but would like to know if we can bid for some items, not all. Response: Please refer to response on question No. 13. 29. Is CHINA and acceptable Country of Origin? Response: Based on the latest foreign policy restrictions, China is no longer an ineligible country, however for this specific procurement, geographic code is ?000? USA only. 30. About Dispensa procedures, we want to know if exported products delivered F.O.B. Tegucigalpa, are tax exempted? Response: Dispensa procedures are made through local custom brokers approved by the American Embassy in Honduras, therefore, exported products are tax exempted. Due to time frame, no more questions regarding this solicitation will be accepted.
