Mortuary Services

This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number FA281625Q0001 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-05. The requirment has been set-aside for 100% Small Business and the associated NAICS code is 812210 and the small business size standard is $7M. Attached is an SF 1449 that contains the list of applicable FAR provisions and clauses, Mortuary Services Performance Statement (PWS), Service Contract Act Wage Determination, Department of the Air Force Instruction 34-160, Attachement 2 Air Force Cemetary Organization Inspection Checklist, and Mortuary Services Price List Template. The PWS, which includes the applicable Appendicies, detailing the Government's requirement for Mortuary Services is attached as an Adobe PDF document. The government anticiaptes the establishment of not more than three (3), five (5) year (Base Year, and four Option Years), Blanket Purchase Agreements to offerors that meet the evalation criteria. Please utilize the attached Mortuary Services Price List Template for quotation submissions. Provision at 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition. Provision at 52.212-2, Evaluation-Commercial Products and Commercial Services applies to this acquisition and the specific evaluation criteria is included in paragraph (a). When offerors submit a response to the solicitation include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services, with offer. Clause at 52.212-4, Contract Terms and Conditions-Commercial Products and Commercial Services, applies to this acquisition.
