Middleton Memorial VA Hospital, 2500 Overlook Terrace, Madison, WI is seeking potential sources capable of providing all personnel, equipment, supplies and pharmaceuticals to provide hemodialysis, peritoneal dialysis, continuous renal replacement therapy (CRRT), and plasmapheresis on an as-needed basis, 24/7, 365 days a year to include federal holidays. All treatments will be performed at the VA hospital. The applicable NAICS code is 621492 with a size standard of $38.5 million.
