The Northern Indiana Health Care System, 2121 Lake Avenue, Fort Wayne, IN 46805-5100 intends to negotiate a sole source acquisition contract with VASAMED INCORPORATED, 7615 GOLDEN TRIANGLE DRIVE, SUITE C, EDEN PRAIRIE, MN 55344-3733 pursuant to FAE 6.302-1. Only (1) source will satisfy agency requirements. The contractor will provide the following equipment and components: Supplies must be compatible with equipment to give correct test results. 1 EACH SENSILASE SYSTEM: FULL SYSTEM WITH ROLL STAND CMR 43F 1 EACH IN-SERVICE TRAINING CERTIFICATION PROGRAM 1 BOX LSA-PLACEMENT GUIDES 1 BOX MICROCUFF LINERS 1 BOX SMALL CUFF LINERS 1 BOX MEDIUM CUFF LINERS 1 EACH 24 MONTH EXTENDED WARRANTY This notice of intent is not a request for quotations and no solicitation or RFQ will be issued. Any responses to this notice must show clear and convincing evidence that the competition would be advantageous to the Government Interested parties my identify their interest and submit information in response in this notice.
