65--Picture Archiving and Communication System Lease (PACS) (Atlanta,GA/Tuscaloosa, AL)

The Department of Veterans Affairs, VISN 7, Atlanta VA Medical Center, intends to negotiate a sole source contract under the authority of FAR 6.302-1, with Philips Medical Systems to continue providing lease of a Picture Archiving and Communication System (PACS) for radiological procedures in both Atlanta and Tuscaloosa medical facilities. Proposed contract term is one year, pending acquisition of a VISN wide PACS. This notice of intent is NOT A REQUEST FOR COMPETITIVE PROPOSALS. However, interested parties may identify their experience in providing this equipment by submitting the following information. (1) STATEMENT OF CABABILITY: Submit a brief description [three (3) pages or less] that shows your company can provide the equipment/system. Include past experience in providing the equipment to the VA, other Government (Federal or State) agency, or for a private medical facility. Please specify your ability to deliver as stated. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS relative to NAICS 532490, Other Commercial and Industrial Machinery and Equipment Rental and Leasing (size standard $7M). : (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm.
