LASIK EYE SURGERY LICENSING AND MAINTENANCE This is a notice of intent of the Center for Health Care Contracting (CHCC), 2107 17TH Street Suite 68, Fort Sam Houston, Texas 78234-5068 to award a purchase order for non-personal services for the Carl R. Darnell Army Medical Center located in Fort Hood, Texas. The contractor will provide licensing and maintenance support for cor neal refractive surgical procedures using the LADAR Vision 6000 equipment. This is a sole source procurement. The service will be provided by ALCON RefractiveHorizons INC. 6201 South Freeway Fort Worth, Texas 76134-2001. No solicitation document exists and requests for such document will not be considered. Telephone responses will not be accepted and any correspondence must be in writing. The performance period will be 2 April 2007 through 30 September 2007. Note 22 Applies.
