THIS NOTICE IS FOR INFORMATIONAL PURPOSES ONLY UNDER THE PATIENT PROTECTION AND AFFORDABLE CARE ACT - PREVENTION AND PUBLIC HEALTH FUND The Centers for Disease Control and Prevention (CDC), National Center for Chronic Disease Prevention and Health Promotion, Division of Adult and Community Health intends to issue a modification to Contract 200-2008-28054- National Opinion Research Center (NORC) to add an additional 3,680 (2,000 telephone surveys, 1,400 self-administered questionnaire mailings, and 280 in-person interviews) interviews in the Risk Factor Survey. This is not a request for competitive proposals, nor is a solicitation available.
