REQUEST FOR INFORMATION The Department of Commerce (DOC), National Oceanic and Atmospheric Administration (NOAA), Eastern Acquisition Division-Kansas City (EAD-KC) is requesting information for on-site nursing services for four NOAA health units located in Silver Spring, MD; College Park, MD; Germantown, MD; and Seattle, WA (with an option to add a location in Boulder, CO). The Period of Performance is from the date of award of a contract award through 365 days thereafter, and two, twelve-month option years. This Request for Information (RFI) is issued to gain knowledge of the marketplace by identifying companies interested in quoting on these services, their business size, their capabilities, and pricing information. Potential offerors interested in this requirement is asked to submit the following information based on the information provided in the attached Performance Work Statement (PWS): • Company Name, Address and Contact information; • Specific Business Size based on a NAICS code of 621399 (i.e. Large Business, small business; veteran owned; women owned; etc.); • A price schedule that includes hourly rates for nurses to provide the services described in the attached PWS, per location, per year; • Separate pricing for the list of services (per service) in section 16.0 of the PWS; • Information to support the contractor's capabilities to perform the services described in the attached PWS. Include information on experience held by the contractor in performing similar services; brief plan to manage on-site services; examples of similar contracts that the contractor has performed under, etc. It is requested that the information requested herein be submitted to the attention of
