65--Provide Normal Saline Solution to Home Dialysis veteran patients. One year period, no options

Contractor shall provide Normal Saline Solution (NSS) as herein specified; and deliver to the home residence(s) of designated veteran patients of the VA Pittsburgh Healthcare System (VAPHS) who receive dialysis treatments at home. Product description is 0.9% Sodium Chloride Injection, USP, 1000 ml bags. Currently VAPHS has five (5) patients in the home dialysis program, all located within Pennsylvania. (It is possible that future patients in the home program could be located in Ohio or West Virgina, in addition to PA). The exact number of patients may vary each month, and it is expected that delivery order quantities will be adjusted as needed. Each patient requires delivery of 40 bags of NSS for a 4 week period. The contract term will be 6/14/10 or Date of Award if later; continuing for a 12-month period. The total monthly estimated quantity is 200+/- bags, total annual estimated quantity is 2400+/- bags. Contractor must be JCAHO accredited and must be HIPAA compliant. In its response to this RFP, Contractor shall provide all-inclusive pricing (FOB Destination) and specify shipping method and case packaging. Shipping method must be secure and traceable. Award will be made to the Offeror who is responsive to the requirements above, and provides the lowest overall price.
