DESC: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format listed in FAR Subpart 12.6 as supplemented with additional information included with this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) Solicitation FA5270-06-T-0027 is being issued as a Request for Quote (RFQ). (iii) This solicitation incorporates clauses and provisions in effect though the Federal Acquisition Circular (FAC) 2001-07. (iv) The Standard Industry Classification (SIC) for this acquisition is 7699 – Repair Shops and related services and the North American Industry Classification System Code is 811219 – Diagnostic Imaging repair (v) Model – Advantex, Nomenclature: Radiographic Unit, Serial No.: 62889ES1, Power: 480V, 3 Phase, 60 cycles, Control: 120V, 1 Phase, 60 Cycles. Repair work shall be accomplished by General Electric certified staff and must be in compliance with applicable radiation safety procedures. Contractor shall repair x-ray machine to manufacturers specifications and dispose of any items replaced. After installation, unit shall be tested for operational requirements in conjunction with the U.S. government designated x-ray technician prior to acceptance and payment of contract. (vi) All deliveries shall be FOB Destination Kadena AB, Okinawa, Japan. (vii) Provision 52.212-1, Instruction to Offerors - Commercial Items applies to this acquisition. Evaluation will be based on cost and the ability to meet the requirements. (viii) Offeror shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. (ix) The clause 52.212-4 Contract Terms and Conditions - Commercial Items, applies to this acquisition. (x) The clause 52.212-5 Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. (xi) The clause DFAR 252-204-7004 Required Central Contracting Registration applies to this and all future acquisitions. (xii) This is a sole source acquisition for contractor to provide all transportation, tools, equipment, labor and parts to repair medical X-ray system as listed in item (v). The contractor shall hold a certificate of X-ray training from General Electric to include repair and calibration.
