The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Hub Zone, 8(a), small, small disadvantage, or service disabled veteran owned and veteran owned small businesses) relative to NAICS 532291 (size standard of $7 Million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published in the FedBizOpps website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement, in addition to responding to this sources sought announcement. The Department of Veterans Affairs, Alexandria VAMC is seeking sources to provide home mechanical ventilation services to beneficiaries of the Alexandria Veterans Affairs Medical Center. The service area includes eligible and entitled beneficiaries residing in the following Louisiana parishes: Vernon, Rapides, Avoyelles, Beauregard, Allen, Evangeline, St Landry, Calcasieu, Jeff Davis, Acadia, Lafayette, St Martin, Iberia, Vermilion, Cameron, Natchitoches, Grant, Winn, La Salle, Caldwell, Catahoula, Concordia Atascosa, Bandera, Bexar, Comal, Guadalupe, Kendall, Kerr, Medina and Wilson. Contractor shall provide a delivery and set-up of ventilator and related equipment systems in beneficiaries home; preventive maintenance and monthly service, repairs, and rental of ventilator systems to include all parts, labor, and expandable material or supplies and travel costs associated with the equipment. Equipment used in this requirement may be Contractor owned or Government Owned depending on availability of equipment. The Contractor will have a Credentialed Respiratory Care Practitioner available, with a pager, and mobile phone, 24hrs a day, 365 days a year for these services. The intended contract period will be for a base period and four, one-year option periods, which is to begin on or about May 1, 2010.
