D -- Graphical User Interface - QBANK Database

The Centers for Disease Control & Prevention intends to negotiate on a sole-source basis with Swan Solutions, 4903 Sudleys Choice, Harwood, MD 20776 (small business) to continue development of a database system critical for storing & accessing QBANK, the Questionnaire Design Research Lab (QDRL)of researched questions and their documentation on the NCHS website. The contract also encompasses completing enhancements as well as system management & technical support. Swan Solutions has developed this system from its intitial developmental stages to its current production stage and bringing in another contractor would not provide the efficiencies that are provided by having the QBANK developer continue in the process. All responsible sources may submit a quotation which shall be considered by the agency. Anticipated award date is June 1, 2005.
