INTRODUCTION: The Memphis VAMC Pathology & Laboratory Medicine Service is requesting a base year + 4 option years contract for services for Liver and GI Biopsies for gross preparation, stains, microscopic examination and consultation services. The requirements and performance of this contract shall be in accordance with Supplies or Services Price/Cost and Special Contract Requirements. Contractor shall provide laboratory; pre-analytic processing as defined in its laboratory user manual; analysis, reporting of analytic results, and consultation regarding selection, collection, transportation and result interpretation. 2. RESPONSIBILITIES OF CONTRACTOR'S LABORATORY: The Contractor shall provide personnel to perform as required the following tests referred by Memphis VA Medical Center, 1030 Jefferson Avenue Memphis, TN 38104 1. Contractor shall perform tests as required by VA Medical Center. 2. Test will be shipped to Contractor via Courier service and have a 3-day turnaround time after received by Contractor. New medical testing shall not be introduced without prior recommendation and approval of the Contracting Officer and Contracting Officer's Representative (COR). The Contractor shall provide advanced notification of test changes, additions and deletion one month in advance. The Contractor shall provide the scheduled days of testing for the different test procedures with turnaround times. The Contractor shall provide single-site (one lab) specimen testing for easy specimen tracking. Date and time specimen collected. Date and time specimen received at Contractor s lab. Date the test was performed and specimen type. Results of specimen testing including any interpretation and/or analysis of the test results. Name of the individual performing the test. Outline of testing protocols, reference ranges and abnormal results shall be flagged. Note of any comments related to the specimen testing as deemed important to be validity of and/or interpretation of the test results. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE QUOTES.
